Request for Reimbursement & Payment Authorization
Use this form if you are seeking reimbursement for an approved expenditure spent on behalf of the PTA. Contact the Committee Chair or Treasurer if you are unsure of the budget.
After you have filled out this form, please drop it off in the “Treasurer” box located in the school’s front office.
You must attach ORIGINAL receipts or invoices. Copies of shopping carts will not be accepted.

Cash Verification
Use this form if you collected cash or checks at a PTA-sponsored event.
Form Notes
Unit Name: Write “Anaheim Hills Elementary PTA”
Activity: Write the activity that generated the funds (e.g. Book Fair, Spring Carnival)
Requirements
You MUST count all cash with another PTA member. Keep all cash in a cash box. If monies cannot be counted immediately following the end of the event, monies should be kept in the safe.
Ensure checks are made payable to “Anaheim Hills Elementary PTA”. In the memo line, it is recommended that the oldest student's name and teacher's name is written.
When tabulating checks, write down each check number and check amount on the form. You MUST run two calculator tapes on all the checks.
You and another PTA member must sign the form and notify the the Financial Secretary of the deposit.

Conflict/Whistleblower Form
If you are an Executive Board Member or a Committee Chairperson, you must fill this out and return to the PTA Treasurer (Treasurer@ahespta.org) at the beginning of each school year.

Committee Report Form
A committee report for all PTA activities should be submitted 30 days after each PTA activity. Attach any detailed information as requested or needed. Report to be filed with the Executive Board, Committee Chairperson, and others if requested.

Photography Release Form
A PTA photography release form should be used when you want to publish or share photos of students or events, ensuring you have permission from parents or guardians, especially if the images will be used for promotional purposes. This helps avoid legal issues related to copyright and privacy.
